After a few edits to the desktop version of my campaign landing page, I notice the opt in form disappeared from the mobile version. Can anyone help me figure out why? link is: http://perq.com/ford-direct-compatibility/

Did you check to see if the element is hidden on mobile?
I think this may have happened to me before - once I published everything was all good.

Thanks Stefano, I found out what was going on. There was a transparent box on the theme (used as a placeholder for where video would go). The form was getting hidden behind it. Once I moved it, everything was fine. :)
