Pop up form for click through page?

Hi, I have created a click through landing page that I’m pretty happy with. My CTA button on that page is asking potential patients to schedule an appointment. Short of putting a scheduling form on the page (which I would rather not do), is there a way to set up a form that I can link to from my CTA button? How would I go about doing that? Thanks for your help!

Need help with some simple coding please!

Need help with some simple coding please! I have added a bit of html into the header of one of my pages to link to my website. The problem I’m having is I can’t figure out how to have that link open in a new page. Here’s what I have: a href=“http://www.mysite.com”> Visit our Website <> removed intentionally I’m guessing there is probably a fairly simple solution, but I can’t find it. Thanks so much for the help!!
