Hi, Can somebody help to create a custom script to make this page mobile optimized with similar form design as in the desktop version?

Hello,
The Unbounce builder has everything you need to create a mobile version of your landing page. There is no need for a script if all you need to do is organize page elements and content between desktop and mobile.
I suggest you make good use of the Mobile option in the builder. Located at the bottom-right. You can use that mobile specific builder to move page contents independently from the desktop version. Let me know if you have any other questions, I’m happy to help!
