Adding a phone extension field next to the phone number field without making the entire form two columns

Hi there guys, I’m looking for a way to add a field for a phone number extension next to the phone number field without having to make the entire field two columns. What would be the best way. Does anyone have any code or anything? test page: http://emergetms.com/test-page/

Google Event Tracking Not Working

Searched the forums but haven’t seen anything recent on this. I am trying Google Analytics Event Tracking on our buttons with no sucess, for now I’m just working on the first ‘Schedule a Demo’ button on our site. But it’s not tracking. I have put the following script in the Javascript 2 menu with the correct ID. Any help would be greatly appreciated.
