Include LiveChat Inc. Live Chat functionality on Unbounce Page

We utilize LiveChat Inc’s LiveChat across our site and I was wondering how I would include this on our unbounce page.  I put the code into the JavaScript area but it doesn’t seem to have worked.  Here is the code snippet, And here is the page that we’re utilizing. https://bonfyreapp.com/paid-search-lp-test_sh/ Any help is much appreciated. Mystery Box on form field label

When we create a form in unbounce the First Name field has a strange box around the field label that we can’t figure out how to eliminate. See the attached screenshot for an example. It’s happening across all of the pages we have a form.
