My Form Looks Like it May Have Broke

1: What challenge are you currently trying to solve? Give as much detail as possible We built our landing page to include a Form that exports to our CRM platform. We use the exact same for virtually everything. In this case - it looks like the form completely broke. We made no additional changes or variations - so we’re a bit perplexed and worried about the next landing page: 2: How are you driving traffic to your page? Paid and organic traffic 3: What is your conversion goal? As many form fills as possible 4: Provide a link to your published landing page / convertable: ex. http://www.landingpage.com/ http://www.jwce.com/productdestruction/

Forms loading in safari, but not in any other browser

Our forms some how are not loading in Chrome, Firefox, IE, & only work in Safari.
