Passing IP Address & form data to URL

Hi Everyone I’m submitting data from a form to an external service via a URL. I also need to pass the users IP address through this form. Is this possible? If so how can I do this?

I can't click into fields

Hi Everyone I created a form, and for some reason the user can not click into three different fields. Fields are: Last Name Zip Code Phone Number Any ideas what the issue is? http://unbouncepages.com/nj-teachers-lounge/

Custom Code In Form Section

Im working with a vendor that need the code below to be inputted into my form section. Anyway I can add custom code to the form section in unbounce? <label><input type="hidden" id = leadid_tcpa_disclosure"/> By clicking the Request my quote now button above, I understand that I may be contacted by Plymouth Rock. I agree to receive calls about insurance products (which may be auto-dialed, use artificial or pre-recorded voice) from Plymouth Rock or their agents to the number I provided above.
