Form not showing in preview?

I’ve added a regular form to my landing page and all seems to be working fine in the page builder, then on preview the form seems to have vanished, any ideas what would be causing this? The form button shows, it’s just the insert text fields and GDPR checkbox that appears to be missing. Cheers!
