The unbounce support guy (Mark) told me to try contact someone from the community that might be able to help me figure out this code.

Basically I need a form that I can get the basic data from a person… name, phone, email etc. When they choose one of 2 products hidden fields will become visible with questions regarding their choice. Is there anyway you could help me with this? I would really appreciate it!
