I don’t have experience with the form builder you are using and I am not a coder so I can’t say.

I did just finish the form optimization course on Conversion XL yesterday and from what I take away, not including the select all button should not harm your forms conversion rate.

Personally, I would combine thigh’s and make an undecided box. The undecided box will let you know from a sales perspective whether someone still has questions or if they aren’t 100% sold on the idea that they want/need cool-sculpting.
