What I wanted to happen is when the second cta is clicked, it will open a window/modal full screen with the form to input request.
Is it possible and could you point me to a resource for it? Thanks!

The preview link you’ve provided can only be open from your account or from people added to your account.
You need to publish your page and provide a direct link if you want us to take a look at the page.
However, based on your question, you might want to take a look at Lightbox element (the popup windows/moduls) within the Unbounce builder. You can also set the CTA to open such a lightbox.
The only limitation is that you can’t make the lightbox full screen. You need to set width/height.
