Need Javascript Help. Looking for someone to program a simplistic “date of birth” field for my contact form that is easy to use and collects the data in a consistent format. Current field is open ended and users fill data in whatever format they desire.

Your suggestion with the “Date Picker” has really helped. I have another question for you. When I go to the theme site for Date Picker, it looks like they have an “auto complete” function. Are there instructions somewhere for implementing an auto complete code in my Unbounce form? Thanks.
