Hi,
I have a form on the main page, but I would like to add form on the light boxes too.
The only way is by HTML? is there a place where I can find a similar looking form to this of unbouce? (the boxes, the buttons, etc)

Unbounce pages only support 1 form at this time, but there is an easy workaround.
- Create a new page with a form
- On your main page, select your button(s) and hit “Go to URL in Lightbox”
- Paste the URL of your new page
You can find the “click action” on the right panel.

Nice, can I change the boxes to be horizontally oriented and not vertical?

Are you referring to the lightboxes?
Absolutely. You can set the height & width to match your new page.
No, of the form itself.

I found a script just for 2 fields and I have 4 including the submit button
