I have the below javascript on my form so that when they select a state, the locations that we have in that state appear. Is there a way to make it so that whatever state/city they choose, a certain confirmation dialog pops up? (i.e. what I would LOVE to make happen is that they choose a state, then city, hit submit and then the dialog box shows the number/email for that location).
Is that possible?

There is a script posted in the community here that has an option for conditional fields based on state/city… It sounds like this is what you need.
