Does anyone know how to create a scrolling form bar that sticks to the top of the page?

I’m looking to create a page similar to the link I have added below. When you scroll the page the form and bar become sticky and as you can see the “SCHEDULE SERVICE” Turns into a telephone number.

Hey Stefano

Thanks for the tip. So I was able to make the box appear at the top, but the form won’t do up there. I added the ID for the box and the ID for the form but that did not work. Is there a way to have the Box and the menu disappear from the position it’s in now and appear on top?
