So I’m creating a form where fields become visible after the above field is completed.
Here’s how I want it to perform:
What would also be great is having the form box extend with these actions.
I have the script that allows a form field to become visible after another field’s completion. I just don’t know how I can use that script more than once in a cascading order.
Any help would be much appreciated!
That sounds like a pretty custom code job. Have you considered using a third party form?

In this case, these customizations might be easier to accomplish with a third party form builder like Wufoo. I won’t go into too much detail, but forms are something that we’ve been discussing in great detail here at Unbounce lately. 😉
